Born on March 12, 2008, in full term via Caesarian Section.
Birth weight: 4.0kg (8.8lbs); Length: 54 cm.
Admitted in the NICU for 21 days
Intubated in her 3rd day/extubated on the fifth day.
Diagnosis after birth:
Congenital cataract on both eyes.
Cleft palate
Persistent Pulmonary Hypertension of the new Newborn (PPHN)
Patent Foramen Ovale (PFO)
Patent Ductus Arteriosus (PDA)
Renal Artery Stenosis, mild on the right.
Pneumonia on the right Lung.

PDA 0.5cm pulmonic end with systolic-Doppler signals across it
Cardiac valves are structurally normal but slightly echogenic (thickened) with trivial to mild tricuspid, pulmonary and mitral regurgitation.
Est. pulmonary pressure 50mmHg
Findings suggestive of persistent pulmonary hypertension of the new born.

Follow-up study shows persistent septal drop-out at nid-atrial septum with dominant left-to-right shunting across it; consider stretched PFO (0.92cm) versus atrial septal defect (secundum type).
Mild shunting across PDA.
Structurally normal cardiac valves with slightly thickening; tricuspid, and mitral regurgitation.
Decreased pulmonary artery pressure to 30-40 mmHg.
Right atrial and right ventricular dominance.
Good left ventricular size and systolic function. Ejection fraction is 68%.
No pericardial effusion.
No coarctation of the aorta.

Intact interventricular septum.
No residual shunting across PDA.
Structurally normal tricuspid, mitral and aortic valves with trivial tricuspid and mitral regurgitation.
Right atrial and right ventricular dominance.
Normal sized left ventricle and good systolic function. Ejection fraction is 56%.
No abnormal masses.

May 19, 2008 a whole body CT Angiography (with dye) was performed and Renal Artery Stenosis was ruled out nothing was found to cause the hypertension.

OROGASTRIC TUBE was inserted to aid feeding and to increase Breanna’s weight. Breanna’s birth weight was 4.0kg. upon insertion of OGT she weighs 3.25kg. The OGT was later removed on June 14, 2008. A special needs feeder (Haberman cleft palate feeder) was used up to the present.

Heart checkup 2d Echo @ SPH – PDA was closed, PFO insignificant change in size.
Orbital Ultrasound @ DDH on both eyes to determine the extent of the cataract.

Follow-up study shows persistent moderate-sized atrial septal defect (1.7cm), secundum type, with dominant left-to-right shunting across it.
Persistent patent ductus arteriosus (0.27cm pulmonic end) with continuous Doppler signals across it.
Structurally normal mitral, tricuspid, pulmonary and aortic valves with mild tricuspid and mitral regurgitations.
Normal pulmonary artery pressure by acceleration time
Right atrial and ventricular with systolic function. Ejection fraction is 68%
No abnormal masses
No pericardial effusion
